As Heather closes the bookstore, a figure in a skull mask stabs her. She flees to the Halloween shop, but the killer, Skull Mask, finds her. Heather discovers that the killer has murdered several people in the mall. She unmasks the killer as Ryan, who is then shot by Sheriff Nick Goode. Heather dies from her wounds.
Shadyside vs. Sunnyvale
The intro reveals that Ryan killed eight people. The story focuses on Shadyside, a town plagued by periodic killing sprees and economic hardship, in contrast to the prosperous Sunnyvale. Deena, a young girl, is writing a letter to her ex, Sam. Her brother, Josh, researches the Shadyside killings online, believing Sarah Fier, the Shadyside witch, is responsible. Vigil and the Car Crash
Deena meets her friends Kate and Simon, where Kate reveals she's dealing drugs again. Due to the recent killings, Shadyside High attends a vigil in Sunnyvale. Deena sees Sam with her new girlfriend and becomes jealous. Deena and Sam argue about their breakup, caused by Sam's move to Sunnyvale after her parents' divorce. A fight breaks out between Shadyside and Sunnyvale high schoolers. On the bus ride home, Sam's boyfriend, Peter, throws bottles at the bus. Deena and Kate retaliate, causing Sam's car to crash. Sam bleeds onto the ground, and Deena, Simon, and Kate rush to help her.
Hospital Horror
The next night, Deena sees news about Ryan Torres being the mall killer. A figure in a Skull Mask costume appears in her yard. Kate and Simon are at a babysitting job, and Deena calls to warn them about Peter. They hear a window breaking and find the Skull Mask figure holding Simon's bloody shirt. They decide to confront Sam at the hospital, where they learn from Nurse Beddy that Peter has been there the whole time. Skull Mask appears at the hospital, and Deena, Sam, Simon, Kate, and Josh flee. They find the receptionist dead and Nurse Beddy stabbed. The killer is revealed to be Ryan Torres. The teens escape in a stolen ambulance.
Unbelievable Truth
Martin, the mall janitor, is accused of vandalism by Sheriff Nick Goode. Sam and Deena try to report the events at the police station, but officers Kapinski and Goode don't believe them. Simon is attacked by a lady with a shaving knife, who they later find out is Ruby Lane. Deena shoots the lady, but she gets back up. Back at Deena's house, Josh explains that Ruby Lane was a Shadyside serial killer from 30 years ago. Josh shows them articles about other Shadyside killers, including the Masked Killer from Camp Nightwing in 1978 and Pastor Miller from 1666.
Sarah Fier's Curse
Josh explains that Sarah Fier is turning people into serial killers to avenge her execution in 1666. The legend says she cut off her cursed hand before her death. Kate notes that the dead killers are rising to target them specifically. Sam reveals she saw visions of the witch the night of the accident. The teens investigate the accident site and discover Sarah Fier's grave. They try to restore the bones, but Sam has another nosebleed. The Camp Nightwing killer appears.
The Plan to End the Curse
The teens realize the killers are only after Sam because of her blood on Simon's shirt and Deena's shoe. They plan to use Sam as bait. At school, they change clothes to remove Sam's blood. Deena, Simon, Kate, and Josh wait for the killers while Sam hides in the bathroom. Skull Mask, Ruby Lane, and the Camp Nightwing Killer enter the bathroom. The teens detonate explosives, but the killers revive. Josh finds an article about C. Berman, the sole survivor of the Camp Nightwing Massacre who claimed to have seen the witch. They learn that C. Berman died for a few minutes before being revived. They decide to kill Sam and bring her back to life.
Grocery Store Showdown
The teens escape to Simon's grocery store to re-enact Timothy's revival. They plan to overdose Sam on drugs and revive her with an EpiPen. Josh, Simon, and Kate smear Sam's blood on themselves to distract the killers. Deena stays with Sam to help her take the drugs. Sam vomits the pills, but Kate intervenes. Deena decides to drown Sam in a lobster tank. Skull Mask kills Kate with a bread slicer. Ruby Lane chases Simon, and the Camp Nightwing Killer attacks Josh. Deena drowns Sam.
Resurrection and Aftermath
The killers disappear, and Sam is dead. Josh and Deena inject Sam with EpiPens, but she doesn't revive. Deena performs CPR, and Sam comes back to life. At the police station, they lie about the events. Josh helps Martin escape jail. Sam's mom picks them up, and Sam decides to stop hiding her relationship with Deena. Deena receives a call from C. Berman, who warns that the witch makes the rules and it's not over.
The Possession
In a cave, Sam's name appears under Ryan Torres' name. Sam, possessed, stabs Deena. Josh is unaware, listening to music in the basement. Deena shows Josh that Sam is tied up and possessed. Deena promises to save her, ending the movie.
