Introduction: Facing Kidney Failure

Dr. Shafiq Cheema introduces the topic of managing the diagnosis of kidney failure or end-stage kidney disease. He acknowledges the fear and shock that often accompany such news and aims to provide a step-by-step guide to navigate this challenging situation. The initial advice is to remain calm and adopt a structured approach to understanding and addressing the condition.

Step 1: Understanding Your Condition

The first step involves consulting a doctor to thoroughly assess and understand the specifics of your kidney condition. This includes determining the extent of kidney function, identifying the underlying causes of failure, and recognizing any associated symptoms. Symptoms can be gradual, such as changes in complexion, breathing difficulties, nausea, vomiting, or bone pain, but some individuals may experience minimal symptoms. Understanding your GFR (Glomerular Filtration Rate) and discussing next steps with your doctor are crucial at this stage.

Step 2: Exploring Treatment Options

The second step focuses on exploring available treatment options, which the doctor will explain based on the GFR. These options include hemodialysis (center-based), peritoneal dialysis (home-based, via a catheter), preemptive kidney transplantation (before complete kidney failure), and a conservative approach involving diet and medication to manage the condition. The importance of making an informed decision about treatment is stressed, as rejecting necessary interventions like dialysis can lead to severe health consequences.

Step 3: Implementing the Chosen Treatment

The third step involves actively implementing the chosen treatment plan. This may include preparing for hemodialysis by creating a fistula, planning for PD catheter placement for peritoneal dialysis, or starting the transplant process. The goal is to transition smoothly into the chosen treatment to avoid or minimize the need for dialysis. The doctor shares a success story of a patient who underwent preemptive kidney transplant and never required dialysis.

Step 4: Making Dietary Changes

The fourth step involves making necessary dietary adjustments to manage accumulated potassium and phosphorus due to reduced kidney function. This typically includes a diet low in potassium, phosphorus, and salt. Water restriction may also be necessary in cases of excessive swelling and reduced urination, but it is not always required. Consulting with your doctor about specific dietary needs is essential.

Step 5: Maintaining Emotional Strength

The final step emphasizes the importance of emotional resilience, patience, and faith in the face of illness. The doctor advises against succumbing to fear, depression, or denial, which can lead to seeking unproven or harmful treatments. He recounts a case where a patient abandoned prescribed medications for alternative remedies, resulting in a severe health crisis. Maintaining trust in your doctor and adhering to the recommended treatment plan are crucial for managing kidney failure effectively.

0:02 [Music] Yes, Assalam Walekum. My name is Dr Shafiq Cheema. I am a Diplomate American Board of Internal Medicine and Nephrology. Subscribe, like and share our YouTube channel for more videos and updates. Today we will talk about when kidneys fail , end stage kidney disease occurs and the doctor suddenly tells you that your kidneys are not working. So what steps should be taken at that time, what should be done ? Because I have seen people get very scared, worried , stunned and go into shock. They don't understand. So, in my opinion, there is no need to panic about that. There should be a step wise approach. Step number one should be that you consult a doctor and assess yourself and try to understand your illness, what is the condition of your kidneys? Why did you

0:47 fail? How many percent are working ? What are its symptoms? Do you have any problem or not? It is not necessary that there be any alarm. Like some people do it gradually till they reach the stage of kidney failure. So their symptoms are also gradual. For example, if the complexion of the face changes, then it changes gradually. Over a period of years. Sometimes you don't even realize that you are you because you see it a little bit daily. So if I see it for the first time, I will pick up two people out of 10 whose kidneys I know have failed. The face becomes a little pale. A little yolk comes out. There could be a breathing issue. You might have to think about it. Nausea may occur. Vomiting may occur. There can be many symptoms of bone pain. Some of these may happen and some may not. But

1:34 some people say, Doctor Sahib, we are definitely feeling like urinating. We are also having food. I recently saw a patient who was a little athletic. Have been playing all my life. Also play badminton. He had no idea. His GFR had gone up to 810. When you suddenly get the tests done then I tell you to be thankful to Allah that you are not suffering from any problem because your kidneys are failing or have already failed. Secondly, you want to be safe as well. Some people don't have it. They do n't think. Those whose urine comes normally. So having said that the important thing is to understand the disease. Understand your GFR and see what symptoms you have. Talk to the doctor again about what to do next. Step number two is that the doctor will tell you about its treatment options since your GFR has

2:20 gone below 10. It is not a stage five disease. This is such a alarm. So here you have to get hemodialysis done which is done by going to the center or you have to get peritoneal dialysis done through the stomach which is done by only a few nephrologists in Pakistan. I am one of them. So those who don't do it, they say no no no, don't get it done, so this is a good option. The third best option that I like is preem transplantation where you get the kidney transplant done before the kidney fails. I try to do the same for all my patients. Then whatever is there in that, you start things at that very moment. The fourth option is a conservative approach in which you neither have to undergo dialysis nor transplant. You don't want to do anything. So we try to control it to the maximum through diet and medicines so that you can live a good life for as long as possible.

3:06 Otherwise kidney is a vital organ. If you undergo new dialysis or transplant, death may occur. Therefore, it is important to take a decision after understanding it well. Let me tell you that your creatine is high. Dialysis may be needed. You say no, I don't want to get dialysis done. I don't want to do it. And then you are the one who is pleading when you are unable to breathe at all. So you don’t immediately reject it like this saying that it is not to be done. Illness comes, you have to fight it. You have to understand as the doctor is telling. Have to try. Thirdly, whatever treatment you choose from these. For example, if you choose hemodialysis then you will have to get a slip made in your arm. If abdominal peritoneal is done then we have to plan for PD catheter. If you have undergone transplant then we have to start the workout as soon as possible so that you do

3:52 not have to go for dialysis. I have one patient right now. Recently, I got his kidney transplant done. His name is Sultan Sahab. Prem has undergone kidney transplant and never needed dialysis. This is the best. In this, your health, heart and everything remains better. So, this can be a very good option. But his tests start failing. If there is a family member whose blood group matches, we advise some tests. Patients come after getting tests done. We get the transplant done. Fourthly, some changes have to be made in the diet. Because when your kidney becomes so advanced , GFR is low then your potassium phosphorus accumulates. So we eat foods low in potassium, low in phosphorus , low in salt. In some cases, water is also restricted a little. If there is too much swelling and less urination. But if urine is coming out normally then I do

4:38 not restrict water unnecessarily. That creates more issues. So you should ask this to your doctor. And last but not least, the fifth step is to make yourself emotionally strong. Be patient. Allah is with those who are patient. Illness has arrived. Fight him manfully. To get scared in trouble. There is a bigger problem than that. So you cannot do it this way that you get upset, get depressed, go into shock. Then they went into denial. Then again, he's doing weird things. Some Hakim, Homeopathy, some this or that, you even leave the medicines in it. Recently I saw a patient. He had his three cremations a year ago. I gave up all other medicines and started taking homeopathic medicines. When I came, my BP had reached 200 b 100 action eight. So I asked why did you

5:24 do this? That so and so and so told me to do this, so I did it. I said my kidneys have failed. I am not saying that I should have saved these kidneys of yours, but with proper treatment, these should have lasted for 5 to 10 years instead of six months. So you have lost so much time and now you will need dialysis so soon. So no one should kill any fool like this. Have faith in Allah. I also tell patients that your heart, liver, anything could have failed. He doesn't even have any special transplant. It does not happen in Pakistan. Your organ has failed due to which you may need dialysis. There is no pain in dialysis. People undergo dialysis for more than 30 years. Transplant is possible. Every person has two kidneys. You can undergo dialysis while sitting at home.

6:10 Ok? There is a change in your life. This is a traumatic event. But this does not mean that you should give up all your courage and sit idle. So follow these five steps. Understand the disease. See Alamat. Discuss dialysis treatment options. Choose any one of them. Make changes in your diet and then have faith in Allah and trust your doctor. Thank you very much. [music]
