Reflection of chest conditions

0:01 let me first exit full screen there still okay if I do this can you see me yes okay I okay I will do this you can see the yeah you're moving okay I I do this move to your left left left left one more left yeah yeah yes but you can see the screen right this is correct okay all right so let's do this hi everyone good morning good afternoon and good evening um so I was trained invest medicine and I started to learn uh traditional Chinese medicine

0:49 from year 2018 and I was very very lucky since 2020 I started to learn um modern Tong diagnostic with Dr Yin so it really helped me to understand more deeply about the traditional Chinese medicine today my topic is more about what the what I see on the daily clinical practice and how modern Tong diagnostic help me with each case so it's more from the uh Western medicine point of view so this is a paper in um the journal medical history um in the 19th

1:36 century most western doctor actually look at the patient's tongue um this is a cartoon in welcome library in London you can see the doctor not only look at the tongue he also put his finger on the patient's um wrist to check the pause uh however in the um even they look at the tongue they actually just look at the tongue itself instead of Link any clinical information to the tongue um in the late 20th century Tong um inspection gradually um gradually diminished the um his PO popularity mostly because so there's not much information in the

2:24 medical textbook to describe where to look what to look um how how you link the clinical information to the tongue so we all know the first tongue diagnostic book published in traditional Chinese medicine was year 1341 um so tongue tongue diagnosis in traditional Chinese medicine has been really well developed um on the other side in the western medicine there's actually only one book meanly describe the tongue diagnosis that is in year uh 1888 um we all know um in the all the

3:11 past over over a century there's no more um Western medicine focus on tongue diagnosis even in that book is meanly describ um tongue itself like um not much um not much pathology link to the inter internal organ so in the old time you can see you have to uh draw the tongue picture and uh um to keep it and describe what the main problem and the the corresponding treatment so on the left side the the the picture about the picture is the short description about the tongue and then on the lower part is corresponding

3:58 herbal formula to treat this kind of um tongue um using the herbs um with the universal use of iPhone nowadays we can we can analyze the same tongue across the world like you're in London you're in China here we are all can discuss the tongue um right away after you take the picture so and nowadays it really make the the remote the remote um diagnosis very easy you all uh remember this one right the left side is um Dr yin's um the Holo holographic reflection image of the the

4:45 body and the right side is the internal organ how it reflect on the tongue so you the baby is is the Supine position and you need to remember where the arm where the leg where the hip um is located so uh this is just to give you example this lady I saw her um the first time I went to the platy class uh so she's next to me and she was complaining her neck hurt her neck hurt so after the class I said oh let me look at your tongue so I took this picture so um I was telling her I said you not uh your sleep is not good good you also uh you

5:31 not only has neck pain you also have thoratic spine pain your your middle back also hurt your lower back is okay uh oh by the way um you had the um breast impact so she was shocked how did I know she has breast implant right and you can see on the both on the side here the the left side here especially the left side and the the right side both heart is protruding out and you can also say the color is very very dark with all the um dark dots on both side so you why why is dark because the breast implant is a foreign body right there's no blood

6:17 flow so this is why so I was teasing her I said I don't know other secret um is so it's just by looking at your tongue I got all the information and she said yes her sleep is not good and also I asked her do you have any family history of stroke she said uh her father actually died from uh stroke so today I'm going to uh just uh reveal like the common the common thing we see um uh at Clinic from the head the neck chest um abdominal area pus uh your extremity and also we will look at the cute versus chronic tongue change and also

7:03 how important the tongue color the tongue coding Etc okay first topic is insomnia so the common tongue tongue characteristic of chronic insomnia is a tip is flat but some patients they are very anxious and um very stressful so you also we see the Arrow Head shied uh over the whole tongue and also you see a lot of red dots and the tip is very red um um due to the anxiety from Western medicine the mostly reason for insomnia is stress anxiety and poor uh sleeping environment po habit and also some mental health

7:49 condition you can also have insomnia due to your physical condition um and also certain medicine also can cause you insomnia okay this is a 44 year old male uh he has a um very horrible childhood his uh his mom actually committed a suicide in front of him so you can uh see his tongue tip there's a lot of Red Dot and the Sheep is like Arrowhead so um this suggest his um anxiety and uh also on the tip it is flat that show the the insomnia this is a lady uh 56 year old she uh her

8:35 husband passed away 10 years ago so since then uh she was not able to um to sleep and uh she also started to have drinking issues the left side is year um October October 2021 so that is the first visit she started um to see me and then you can see the chip is very very flat and that suggest his insomnia suggest her insomnia and then the middle one is actually uh almost two months later uh so she finished the treatment by um after two months and you can see the tongue sheep change the tongue color

9:20 change and all the cracks change um so she was able to sleep well she also totally quit drinking and then the right one is actually uh almost two years later so she came back for something else you can see her sleep is still very good and the tong ship keep to be like this instead with a flat um top and this is a example of headache so if you look at a headache most the people the tip is red with those Red Dot this is a 15year old boy who had a concussion n months before his first visit um so he has a lot of pressure

10:06 sensation also feel dizzy especially after exercise so the left side you see the tip is very red even like a purplish around the the tip and also you see the Midland um Dr ma Dr Yin and Dr Sun all talked about this you can see the Midland is is curved it's not straight right this suggest his spun is is not straight there's some scis he's very skinny but he was doing a lot of weightlifting so the the muscle is not strong enough to to support the spine and that caused the um his span actually curled um on the right side this is May 29th this is July T so uh

10:54 about one month and a half after the treatment so I asked him to stop weightlifting do a lot of stretching and uh pay attention to his sitting posture at the school and you see um his his headache is basically gone and you can see the tip and another thing is you see the spa the Midland is much much steter um Now we move to the the neck and the back just like what we saw in the previous case so the midline of the tongue if you you see a crack or protruding area it suggests there's some pathology either for the spine or the soft tissue um that around the spine um especially for those chronic issues you

11:41 will see more of the crack but the case we saw early um he's young and this is a this is pretty recent so you don't see the crack yet so the uh Midland is All U the Midland Bend is used see the in scois some dislocation some dislocation of the spine or the muscle or fascia um the tension cause the like um the the electrical um Pole right if the um the the it is Twisted tilted to one side it means the one side of the tension is higher than the other and by looking at where the the

12:28 location um you can know get a idea is it the cervical spine the thoratic spine the Lumar um you can know where is the pathology and you will also do corresponding examination on the patient this is a um a 61 year old um patient with chronic insomnia and a severe severe neck pain for 15 years so he actually feel a lot of um brain fog with the neck pain and um in the past eight months he it really affected his sleep on the left side is uh his first visit this is March uh 11th you can see the the neck is this area is very very

13:15 tight and um it's like the muscle like a candle uh cramps and also you see he also have lower back pin and you see the lower back is also U with a big crack and then they this is the color is purplish this is May um May 21st um two months after the uh first visit and you see the neck pin is basically resed um he still have some uh like a sleep issue but you see the the neck part is much much relaxed compared to before and the the right one is um after another five six months and you see the color and the shape is much much better and his sleep

14:03 started to get better too this is a 70y old lady with chronic sclerosis um she recent she had a um from T9 thoratic n to S1 she has the fusion and you can see from the right side the xray the fusion is um from the T9 to S1 you also really see it's still very cured so the direction of the of the spine is actually is the same as the tongue you can see the tongue um the the spine tilted to the right the you can see the the mid line that crack is also

14:50 just as a mirror of her spine uh this is a interesting case this patient uh one time during the her visit I saw the left side with the cating you can see the midline is here and the coding is just on the on the left side so I asked her anything wrong with your left side and she said because it's right after holiday she said she was lying on the sofa in the wrong position too long and she um in the past from yesterday she started to have the left side lower back and the middle back pin and you can actually is clearly you know the coating is in the lower back and the

15:35 uh in the middle back and next time when she is back the the left side um back pin is gone and the coding is gone okay so uh Now we move to chest so the organs in the chest cavity the rib or the chest muscle change can all reflect on the on the tongue so just remember the the little uh baby on the tongue so the first top third of the tongue will reflect what's going on in the chest this is a 58 year old female she uh had a stage four lung cancer with um also with the thic and lbar Bone um

16:20 metasis and five years ago um but very fortunate she actually by using the genetic um Target cere y she also has been cancer free in the past three years you can see the whole tongue uh first of all you see this tongue is not um you feel something is wrong this is tongue suggest the patient might have something severe disease and also you see the crack is mainly on the tongue reflection area and this is uh June 29th this is December 21st and you see there's a the tongue color um the crack is all and also the tooth marker all getting uh

17:07 much much better the tongue is also more symmetric that time she just feel like a normal person this is a 57 year old female she present with uh severe dry cough the cough is like a crazy non-stop very deep for three months she thought she had a mold in uh in her apartment um she thought she had a covid multiple test and now those um was positive for covid so when she stick out of the tongue you can see in the blue circle you see he around here there inside is red is like a purplish and protruding out and so you

17:55 see something is wrong in the chest that time I feel I I don't think it's a lung cancer because it's unlikely in the middle but I really didn't know what's going on so I sent her to have xray and um the X3 showed the tumor so they did a CT the nextday which showed is a 15 time 13 time 7 um tumor in the thalamus and later they had biopsy which showed that she had diffused lar B cell lymphoma so she uh had a uh sixth round of chemo uh eight months later she came back for the uh for the neuropathy for the hand neuropathy and that time she is already in remission and then you can see the

18:43 the that big protruding um thing in the chest is um is gone I keep the teeth to to let people realize is the same person so now we move to the abdominal area the T image of patient with digestive issues they will show some horizontal or small vertical cracks in the middle and the lower part of the tongue this is also uh sometimes help you to defrate if it is by is a spine issue or is the digestive issue and of course the upper GI problem will be closer to the top and the lower G eye will be in the middle and the lower part of the tongue

19:29 this is 18y old girl with uh severe anxiety so she started to have digestive symptoms since middle school so she has each time she said after eating she had a stomach cramps she burp and she has a lot of food intolerance um you you see the in the circle this is the upper gii and you can see there's a lot of cracks and also it's sunken and the color is much much darker um in in this area so and also you see her anxiety you see the Red Dot on the tip and you see the tip is red so even without this patient tell you if you see

20:14 a tongue like this you know this patient have the the digestive issues and anxiety this is a 58 year old female with buried esophagus and she also help pancreatic insufficiency and the gastro parisis she is actually also uh using OIC she has diabetes too um so she she her husband is actually a GN uh doctor is a surgeon um so she has been tried everything but from the traditional Western medicine but now those works so she was Waring at least 15 times a week in the past two

21:02 to three years so with those strong um episode you see the deep deep cracks in the upper GI system so you know the the the the gut the the stomach the esophagus and all the soft tissue around it has the pathology and you can see clear from here um this is a 13 year old boy uh he started to have constipation since childhood and um he also have a stomach cramps and uh U whenever he eat something um not right he will have skin hies his ear always red and he told his mom his ear hurt the mom just didn't

21:48 believe him and then um that is actually because his poor gut he has the skin reaction to different food um so you see a lot of um the the redness the whole tongue is red and also you see the the yellow gispy coding and here this is a thick figure coding that show his constipation and uh it's also very watery so this is uh January 17th this is um May 6 so after uh three months or treatment his symptoms is basically went away um no no ear pain anymore more no uh severe uh skin reaction to different

22:34 food this is a 20 year old girl um she started to have blood infection since age three and in the past two years she had she had four episode of pylon nephritis and each time she needed the IV antibiotics um for the pontis and you can see at the bottom of the tongue which is is corresponding to the to the bladder area there's a really very yellow thick coding um so that is show the chronic inflammation in the pelvis so the bladder actually like a living a polluted environment right all the blood flow all the just like a we see outside if is polluted is not clear and the is

23:22 not healthy so the bladder environment is really not healthy that show The Chronic inflammation this is a 20y old uh boy um he was diagnosed osero colitis at the age 16 he tried all kind of medicine so currently uh she he is on the on the uh imun suppressor and also on the oral steroid so uh he had a six to seven times bloody stew and also um cramps every day and and also very very tired so you see there there's a big bigger crack and on the bottom part of the

24:08 tongue this is this is corresponding to the rectal area so I when I saw that I said oh your pathology uh your um your um your um uh osero colitis is meanly on the rectum he said that no um two years ago I had the colonoscopy is also the transverse colon and the descending colon all uh affected I said okay let's see so two weeks later actually he had a colonoscopy and the and the report showed is only the rectum was affected and for people who have autoimmune disease most of them is have a lot of stress so um at age eight uh 16

24:56 I asked him what happened shortly before for that that is when uh his parents started to get divorce this is a two female and the left side is a 45 year old female with heavy period for 3 weeks when I look at the tongue I said oh you have a fibroid uteral fiid and this is shown um in the in the blue circle so she uh went to have ultrasound which showed uh she has two uh uterine fibroid and the right side is a lady present for shingles and then when I see her tongue I said oh you need to have ultrasound and she did and which showed the four uterum

25:44 fibroid Now we move to the shoulder so you remember the little baby uh lying down there the shoulder is on the on the side so this is three case the first one is a 54 year old um male he strin his left shoulder when he was playing ball with his kids so this is only one month's history and you see the left side is very bruised um he has a severe pin on the left shoulder and the left upper arm and you see the color is like a dark bruised and but compared to the right side which is pink so you can clear see the pathology is here and the the second one the middle one is a 17 n year old male and um you see the right

26:32 side is a is like um like a blood stagnation right so he had a right side rotator cup surgery 15 years ago but since the surgery he always feel right side the shoulder pin um so you know this is a chronic um this one the on the right side is a 77 year old male so he has chronic arthritis Rotator c surgery on the right side eight weeks ago so you can even though the left side is more prominent the right side also you can see there's something um going on so the right side after the Rotator c surgery he's feeling better but the left side he has no range of motion he was um he the

27:20 left side he already had the surgery 20 years ago and you know uh for 20 years like a Ral motion the muscle atrop and you see this is Dent dent in for the left side he actually is ready for the left side um shoulder replacement this is a 77 year old female with Parkinson and uh uh she is very very um stiff mainly on the left arm and you see the whole tongue first of all people with Parkinson they're very stiff they they even their face is like the mask so you see the tongue the whole tongue is stiff and the left upper side of the tongue is very streight and the

28:05 lost of the normal Contour compared at the the right side so this is show her left arm um stiffness um so now we see some relatively acute uh changes on the tongue so this is a 70 year old female um this is a December 13th three days later she was she eat some nuts and caused the um she has um accute diverticulitis so she was given oral antibiotics and this is a 60 after the the episode started she was in severe severe abdominal pain when she came in and then you know um you wonder if this

28:52 antibiotics works right obviously it's not the right antibiotics for her so she was actually uh she got a new antibiotics and uh um three actually I have another picture uh three days after that the tongue got much much better and her symptom is much much better too so you see the acute change when the patient have something um ongoing um so this is a lady 51 year old um she had she started to have scis uh since teenager so you see um you see the her spun is is Twisted um tilted to the

29:38 right and then it in the arthritic level and then it go to the left right so this is um this is February uh 16 this is the time is 10:40 this is February 16 time is 11:40 so before the treatment after treatment we have picture and then this is a tongue change um this is February 16 10:34 and this is um this is February 16 11:42 so this is right before the treatment um when she come in I normally when patient came in I normally just take a tongue picture and then we talk about the their condition and then the right side is

30:25 right after the treatment you can see the spine curled really curled before treatment and is a lot of cracks and the right side of the spine is much much steter and the crack is much less because the spine um is more um the the muscle is more in alignment with the spine so it's not much tension then the crack got less this is February uh 22nd 1022 and this is the same day 1110 this is right before the treatment right after the treatment and you see her spine is got even stetter right and then the second time um the the the spine is

31:11 um a little bit straeter more change you see the tongue is not as puffy as before the treatment so this is uh February 16 before first treatment this is February 22nd after second treatment treatment and you'll see the tong significant change because between the treatment patient was also given um herb um for all her conditions um this is a a 85 year old gentleman so his wife um it was my patient and uh he had multiple episode of Syncopy he passed out and he was brought to the ER for eight times they

31:58 did all the test all the Imaging and each time told him um everything is okay they have no idea what cause his simple P so um the left side is on his first visit September 2nd you can see the tongue color is really um is really um like a purplish like a dark even the coding is a greenish so from traditional Chinese medicine the point of view you know the blood the blood is really stagnated and then um this is October 11 so only one month and one week and you see the tongue has a significant change

32:43 this is in year uh 2022 he since that time he never have any Syncopy episode and he continue able to walk one and a half mile every day um so you can see see patient also on this is a very interesting topic so a lot of people nowadays a lot of patient on on anti-coagulation right so this patient is also because of his AIP his on Alquist the anti-coagulation so people say oh you anticoagulation should we use the herbs to help with the blood flow is definitely should because the even they use the anti-coagulation is only make sure the blood don't develop into blood

33:29 clot it doesn't help with the blood flow um efficiency so um for this patient we know that after treatment his blood flow is much much better um so I think uh you know even though traditional Chinese medicine the tongue diagnosis has been for thousands years however Dr Yin really made a huge um huge impact on this area because it not only um incorporate the the traditional Chinese medicine the tongue diag diagnosis but also um integrate modern medicine knowledge especially for the anatomy for the phology so that that

34:17 can let us know more um more about what's going on not only just people who know Chinese medicine understand what what's going on just a normal person can understand um what is going on and uh um it it's not honestly I feel traditional Chinese medicine is much harder to learn than um than Western medicine my PhD is in epidemiology is also much harder than my PhD so um but the Dr yin's method really gave me very objective view uh what means what means um inici what means young deficiency U what means the CH stagnation so that um gave me um like

35:05 a very um intuitive and uh um gave me also not not only understand um you know the the the local area but also the whole body what is going on so I think uh this not only for traditional Chinese medicine doctor but also for all doctors you know even they are C practor or Western doctor um you know Physical Therapy they should all learn this method of course um there's still a lot to research okay so uh my lecture is um basically finished if you have any question uh welcome to ask

35:52 me thank you Dr L anyone you have any question please you can ask dror for

37:18 capture picture pathology captur

38:26 for um

39:12 [Music] for oh uh can someone please translate all of that into English sorry sure okay so um the first question ask how can we got the one patient you know stick out the town for a long time

39:58 um how can we get if that will affect your tongue picture quality so um I was um for me at least I normally take three tongue pictures uh for the front part and one tongue picture for under the tongue you know look at the muscle look at the blood vessel um normally I ask a patient to relax and then stick out the tongue when I see okay stick out the tongue then I will see relax then I capture right away then I let patient that put the tongue back and then I will do the same thing another time and then um three times normally this is unless this patient is really um you know hard doesn't know how to stick out the tongue I will let P practice several times and

40:45 capture more picture then among all the picture I will choose one that really can uh show me the whole U the whole tongue you know the bottom um the the front the the um the whole the whole tongue if it is relaxed I will use that tongue to analyze um the tongue um this is the first question how to get a good tongue picture and the second question talking about um if I only use the acupuncture uh for the for the U scolis uh so um yes the this is this is just by acupuncture the first time that then the second time but between that I also used the Dr Dr y's method gave patient The

41:34 herbal formula so um she is asking how to treat the scis that is um there Dr Chen um 13 um has um I used his method for that so um he can she she can further um got some more information about that but it's impossible for me to explain how to treat that during this leure thank you any other questions um I have a yeah yes I do have a question

42:20 um uh can you describe a little bit about what percentage of confidence you have when you diagnose the tongue so when you're using this method do you feel that you're 70% accurate uh okay so the question is um how how confident I am when at the T when I look at the yeah so so is there some conditions that you feel very confident and some conditions you're confident when you look at the tongue to diagnose yeah so you you never ever um you never ever um assume so when you

43:07 look at a tongue you always you always some of them um some of them you can see oh you know where the problem but you need a a lot of time you need to talk to patient about what the detail uh how they feel so you always combine with the clinical information so this is the example I never I I didn't know this the reason why I look at her tongue so when I saw this girl um in the plati class she was next to me and then she complained the neck pin I I just try to be you know I said oh you you can I didn't tell her I do acupuncture I said you can try some acupuncture she said ah I don't believe it that made me feel

43:54 annoying right so I said okay let me look at the look at your tongue when I look at her tongue I didn't ask anything before I tell her you have you have U sleep issues you have insomnia you have anxiety you have neck pain you have thoratic pain and also you have breast impact so this I can tell 100% but again you don't do that because you always combine just like when you see a Imaging you see a city you see uh you see MRI you still combine with the their Chief complaint to look at their clinical information but Dr Yin this is just like a mirror like what you

44:40 see um this is a you see the the you also know you actually when you see a tongue when you see enough you will know this person's personality so I'm sure Dr y on the on the accuracy is much much High higher than than my but it give you enough confidence first to say okay um in which area or which organ you have problem this is 100% but for those detail you really need to combine with patient clinical presentation and try to also get more uh the detail about what's going on did I answer your

45:28 question um yes partly um can you describe a little bit about how uh confident you feel using the tongue to treat for example digestive condition Gynecology muscular scal uh what's your confidence in those areas and if you could rate in percentage like you feel 70% confident or 80% confident uh I think you know um this is a this is uh if you talk about diagnosis I think the the The Confident is much higher but if you talk about treatment I think is depends on my level of the herbal study so I'm relatively new but

46:15 the good thing is about you have the Baseline of the tongue and you have the with your treatment you see their tongue change and then you know am I on the right direction so if not then you need to adjust the herbs so but I think the diagnosis part the the um diagnosis accuracy is at least 80% okay thank you you're welcome and have any

47:07 question can I ask another question sure um what are some of your favorite uh tongues to diagnose and treat like what are your favorite tongue conditions and tongue presentation and conditions to treat a fury you mean which um like which pathology for for example like insomnia or gut issue or those kind of things yeah yeah I don't think that I think it's you this is so this is what I feel so different from Chinese medicine and the Western medicine so you always treat the whole person so even you see so that's why I

47:52 have no preference if I more like a treat because you even this P person present with um for example with some pis issue you still if you see the you see the um liver the liver cheat stagnation you still treat it so and what I feel that also you you not treat like one part like what western medicine do you treat the whole T you want all all the way everywhere is smooth then that way the patient you don't care about where their problem and they will heal by itself in that case so which persons do

48:40 you find that you enjoy trading a lot because you find that they respond well which sort of persons can you describe yes so people who is a Believer right so a lot of so for me my population is um is all uh I will see um like a 95% is Caucasian so but some of those patient they come they come they uh you know you you during the evaluation you know they are strong believer especially some of them have experience with traditional Chinese medicine but if you see a patient who said oh I had one treatment before um uh

49:26 it doesn't work then you know this is not the right person so people who is a Believer and I truly believe the Mind Body power I think those will have good response okay all right thank you you're welcome any other
