01 Our special guest is an internal medicine expert and the medical director of Heritage Men's Clinic. He has spoken about health care delivery, fertility, sexual health, medical accountability, and the challenges facing Nigeria's health system. Please help me welcome Dr. Rashid Abassi. >> [music] [music]
00 >> THANK YOU. >> [cheering] >> YOU SEE THIS MY >> [cheering] >> SEE, THIS BAD AUDIENCE. WHAT THEY SEE, THEY KNOW they think of breasts, and [ __ ] and vagina, and this sex, and nipple. That's what they like. Yeah, we have to hear. Okay, oh, no problem. Dr. Abassi, I'm so happy that you can't dance. >> Thank you. Thank you. Thank you so much. >> me that they always abusing me that I cannot dance. He dance like a white man. Did you see how he was dancing? Like, ah, you boy. You're even worse than me. Like, I'm so happy that I have somebody who is better than me in the dancing. But, good to have you, Dr. Abassi. >> Same here. Same here. Same here. >> So, as I said, today we're discussing the breast. We're doing the good, the bad, and the ugly. Starting with the good,
46 the breast is supposed to be something that gives satisfaction, especially during sexual activity. Women have complained that men don't know how to handle it. Some are too rough. Some are too um Some are too gentle. They don't They are clueless. They just think it's about rough rough. Others want it rough. You know There's a lot of confusion when it comes to how to enjoy or appreciate the breast. So, as a sexual health advocate, what's your advice on how to handle the breasts? >> Thank you. Thank you so much because, you know, we're in the breast month. >> Yeah. >> And I actually don't think there's anything bad about the breasts. We will only talk about it. >> Okay. >> Breast has two main jobs. One is
32 function, production of milk. So, you may see somebody as a teenager, they are maybe undersized in terms of their breast size. When they get pregnant, the body produces a lot of prolactin anticipating a baby is coming. That baby needs milk. So, they get enlarged. And so, we always encourage that at any point in time they should dislodge. So, once You know, when a woman is having difficulty breastfeeding, we don't like that. So, they have wet nurses all over Europe who will help them to do that. The second function of the breast, obviously, is the sexual. And we need to pay attention to it because it has a lot of function that helps the women. We're in a society where we have self-esteem issues,
17 body dysmorphic stuff. Don't let somebody determine who you are by the size of your breast. As long as that breast is doing the job, it can produce the milk, it can turn you on sexually. And so, those are the two main functions that we should focus on. >> Okay. So, let's talk about the not so good because the insecurity some women have. They grew up with a decent size. And suddenly, it becomes huge and they have it to have specialized bras. Others have it disproportionate. One is larger than one is smaller and it causes a lot of problems. So, it's there, it's functional, it does the job, but it doesn't make them feel good. How do they handle that? >> So, the the the truth physiologically is no two breasts should be the size.
02 >> Okay. >> Just like men's testicles, it's not the same size. One is going to be slightly bigger than the other. And now it depends on how you actually carry what God has given you. It's going to be different for everybody. And men have different specs. Some men like women who are well endowed. Some men like leper. So, it's it's all about and go to where your value is being appreciated. Don't let any You know what I mean. As long as it does those two functions, that's all that matters at the end of the day. >> Fantastic. >> [applause] >> So, given your work in reproductive and sexual health, how do hormonal fluctuations or sexual activity influence breast sensitivity and fullness or its comfort? >> No, this is actually one of the most
48 important thing we should talk about. So, every time a girl goes through menses, >> Yes. >> her breast is going to be exposed to bunch of hormonal variations. >> Okay. >> Which is why when we get to how you prevent things, we always recommend self breast exam immediately after your period. Because then you're going to get used to the knowing what is normal and then finding out what could be abnormal. >> Aha. >> But besides that, when you talk about even the stimulation of the sex, sex is full of nerves and glands. The glands would be the milk production. But the nerves are around the nipple. When a woman is turned on sexually, she has seven other openings in her vagina area that gets lubricated because of that turning on. >> Aha. >> Seven openings. And that opening is
33 essential for fertility. If they're not wet, the man's sperm will die. So, when a woman is turned on just by the nipple, it's supposed to signal to the guy that you can go in. >> See how quiet they are? We're bad people. >> [laughter] >> Okay, so but what can they do to properly care for because I know that especially the the tip of the nipple always gets covered, you know, and there's people are not sure how to clean it or how you know, how should we take care of our breasts? >> So, simplest the simplest avoid all the temptation of what they have online telling you to use some cream and all that. Warm compresses are the best. Your nipple has an opening and it's surrounded by a bunch of nerves. The main thing that turns them on is the
19 release of oxytocin. When you put a warm compress on it like warm towel, you keep the openings open. They are ducts and you don't want it to get blocked. >> Mhm. >> So, even for the milk to be let out, the ducts need to be opened. >> Yeah. >> And then avoid wearing tight bra. Just like we tell male guys, don't wear tight boxers because it can affect the temperature. So, let it be free. Not too tight, not too loose. Because the main issue about women who are well endowed has to deal with back pain. The breast is attached to the chest wall. So, if you find out that you cannot sit up straight just because of your anatomy, then it might be the time you talk to your doctor. >> But some of us are really endowed. We need that wire to hold it. Now, yeah, because you have to Me hey, YOU HAVE TO STRAP IT. SO, WHEN YOU SAY DON'T MAKE IT TIGHT, but I shouldn't make it tight.
05 It has to be tight. If not, it'll be flying all over the place. >> No, no, no. I agree with you because you see it's held by muscles and the muscle has elasticity. >> Mhm. >> So, yes, with time you don't want it to be dragging because it actually going to affect your self-esteem overall. Then when you actually go for fittings and you talk to your tailors, they will let you know because there are different supports. Not everything has to be at the bottom. >> Yeah. >> It could be at the shoulder strap and they'll fit it well for you. >> Then as women grow older, it almost turns to slippers. How can we avoid that? Because a lot of women as they grow older, it's an issue where it's just like this and they're like, "I want it to stand. I want something like you know." So, how do we uh prevent it from falling? >> So, one of the things is throughout your time you want to see as it done the job.
53 What I mean by that is women who got pregnant or had miscarriages and did not let out that milk is going to lead to added weight. >> Mhm. Really? >> that again. So, when a woman is pregnant, your body produces a lot of prolactin. >> Yes. >> One of the major job of prolactin is milk production. >> Okay. >> If for some reason the woman is unfortunate and the baby did not come to term, what's going to happen to that milk production? You need to let it out. >> Yes. >> So, one of our audience has mentioned where when she was breastfeeding, unknowingly to her she was using one breast. >> Yes. >> What they would have advised in like US is just pump the other one. >> Regardless. >> Yes, because both breasts are making production and you don't want one one of your lobules to just get stuck. >> Mhm. Wow. Interesting. Okay.
40 PLEASE CLAP CLAP CLAP. MANY WOMEN DEAL WITH cyclical breast tenderness or general discomfort. What are the main causes of this breast pain? >> So, breast is a sexual organ. You're not supposed to experience pain there. If you start experiencing pain, you need to let somebody know. >> Ah. >> And not only just the whole size of the breast. When you even talking about the nipple, if you start noticing retraction, meaning the nipple looks like it's going in, it's a bad sign. >> Mhm. >> You don't want that. You want to call someone's attention to it. And when it's time for you to examine your breast, go all the way to your armpits. >> Yeah. >> They are full of lymph nodes. They sense what is bad here before anything else.
25 So, even if you just raise your hand up and you feel some pain, please talk to somebody. >> But just somebody who >> So, you talk to your primary care doctor. Or when you have a nurse because women are more comfortable with another woman that understands the anatomy. You just explain to them, is this thing normal? Because it's not all the time you go to mammogram. We recommend mammogram, but a nurse and experienced medical person will be able to let you know, why are you having it here? And in most cases, what happens to the right will happen to the left when there's something wrong. >> Okay. I'm going to come to you guys in a moment if you have questions, but let's come to the issue of the safety of surgery of breast enlargement or reduction because it's become a thing. Now, people who feel that those who have big breasts then ah, please I want it to be moderate. Those who have really small, they want it be bigger. Is it
11 healthy and is it safe? >> So, one of the conditions insurance companies use is when your breast weight >> Yes. >> is causing pathology, meaning it's causing you back pain. Then they will recommend reduction. When it's coming to people who are undersized and they want to enlarge, it's usually not recommended because you are putting an artificial silicone in there. >> Mhm. >> Those silicone is not going to do any functional job for you. Right? And doing the time of production, when they're going into insert it, some nerves may be damaged. So, you are affecting the sexual function of your breast. >> Mhm. And but you can do it once it's Once you are no more doing all these things and you don't have children and they are worried about You don't have sex You don't have sex You can do it now.
56 >> What do you mean? What do you mean when you don't have any more? >> I have a certain Let us not lie to ourselves. At a certain age, women don't have sex again at a certain age. I beg. I am praying for my There is a time where women just Well, let me face my children. That stage, you can increase your You can adjust your breast. >> Yes, yes, you can go for breast Yes, yes, you can go to tighten it up, to move the breast up. Then you might question the essence of that when you are no longer going to allow the man to touch it. >> Yeah. And there's no man I beg, let's move on. Okay, so before I come to you guys, some medical practitioners have said that sucking of the breast can help reduce the risk of breast cancer. Is that true? >> Absolutely. >> Really? >> So, so when your man is when you guys are doing foreplay and your man turns you on, one of the hormones that will be released immediately is oxytocin. Guess
42 what? Oxytocin kills cancer cells. >> Huh. >> [applause] >> So, you need to let your man And it has other benefits. When you have a regular guy that will fondle you all the time, he is one of those to pick up when something is abnormal. >> AHA. SO, THOSE OF YOU THAT DON'T want to go to Jerusalem, please make it available, that part of it available, eh? Okay, let me come to the audience questions. Yes ma'am. >> Good morning, sir. Good to have you again sir. Okay, I could remember when I was working, my late chairman, Professor Deji Femi Pierce.
28 He There's a particular day he called me and he advised me that if I am not allowing any man to touch my breast, that I should allow the guy to touch it, that it helps um to um avoid cancer. That because of the lumps in our breast. So, when the man, you know, it will help. So, sir, I want to ask My question is When I Whenever I want to have my period, I do have pain on my nipple and my breast is always Is it normal, sir? >> Yes, absolutely. And not only around the period, when a woman is also about to
14 ovulate, most women have different signs. Some women may have vaginal dryness, some may have breast tenderness, some may even feel like they're having fever or malaria. These are all consistent because women are going through a lot of hormonal changes in that period. Your follicle will be released, your luteinizing hormone. So, yes, we expect this and that's why we recommend immediately after that, when your hormones might be normalizing, that's the when you want to find out what's going on. >> Okay, one more question. Yes. >> Good morning, Dr. Abasi. Good to have you again. >> Same here. >> My question this morning is that if breast cancer runs in a woman's family, what are the extra steps she can take, or let me say
59 uh the No, no, no, not preventatives, or let me say lifestyle habits >> changes >> she can, you know, >> to reduce the chances of getting >> so that she will reduce the risk of having that cancer at a later years. >> Okay. >> Fantastic. No, I love that question. So, it's breast awareness month. One of the things even in the research community that we just found out that will go to the risk. We know breast cancer runs in family. We just found out now that you need to know your entire family history. This is what I mean by that. We found a correlation between prostate in men and increased chance in women developing breast cancer. Let me explain. So, if you have a
44 brother, let's assume your mom and your aunt no breast issues, but unfortunately, the brother developed prostate problem. If they found out genetically that the prostate cancer has what we call BRCA1 or BRCA2, all of you women in that family, your risk of breast cancer has gone up. >> Mhm. >> And the same thing for the men. So, we need to know our family history. We don't need to be shying away from it. >> Mhm. >> We need to know what happened to grandpa Moses, what happened to uncle so-and-so, >> Wow. >> because of that correlation between the two. >> Okay. Final question from the audience, yes. >> Good day, sir. >> Yes. >> I'm Queen B. >> Queen B. >> I want to ask this question. As a breastfeeding mother, can your husband suck your breast when while you're still breastfeeding? >> Okay.
29 >> Yeah. >> So, so the recommendation is even the time you're breastfeeding, you're still sexual. He can do it. He helps you. Breastfeeding not only helps the baby in terms of the antibodies, it also has been shown to help some women with weight loss. So, you want to do that. If he's comfortable with it, like somebody said, it calms him down, please let my brother have it. >> But why would he be drinking milk, though? WAIT, WAIT. BUT WHY WOULD HE BE DRINKING MILK, your child's milk? >> See, the the >> Ew. Why would he be drinking your child's milk? >> It's not It's not It's not It's not It's not just a milk to the guys. >> Ew. Ew. Disgusting. So, you Wait, hold up. Guys, wait, hold up. Men,
14 your wife is pregnant. Have you actually drank the milk of your child? >> Are you for real? >> That is so disrespectful. Your child's milk? Ew, I can't believe it. Anyways, let's let's just drop it. Let's not be surprised. Like let me just skip that and just move on. Anyway, many women know they should check their breasts, but they don't know how. Can you take us through through how to take care of our breasts, self-examination? >> Yes, yes. Actually, we put a paper out for everybody. >> Can you all bring out your paper, please? Let us see. Bring out your paper, okay? >> The The key with this has to be
01 consistency. >> okay. >> Yes, and we need to be You You want to pick a day after your menstrual cycle to do it because the first thing you want to do is you want to get yourself familiarized with what is normal. >> Yeah. >> How your breast feels. You want to make sure you need only three fingers. And don't forget what is called the perimeter. Perimeter is actually outside of the areola. Come all the way to your armpits. >> Yeah. >> Because that's where the lymph nodes are. Lymph nodes are sensors of your immune system. If they notice any bad cell, they will pick it up first. So, there should be no pain. There should be no tenderness. And you want to start from inside out. Is a little massage. And again, it's not that I'm advocating for men. You can let your men assist you. Honestly, you start from the areola inside out, and you want to actually
48 feel what is normal. For African American black women, we tend to have some lobules. >> Yeah. >> It's not necessarily bad. You get used to it. So, now when there is a change, then that's when you say, "Oh, something is wrong." So, for example, I had a patient that all she presented with [music] was back pain. And it's because she has been ignoring a nodule in her breast that went to the back. So, get familiar with it. Use three fingers, so you are not even feeling for sensation. It's not a sexual turn on at this time. You have your hand at the back, so you can actually expose the old breast tissue. You start from the perimeter. So, under your armpit, you go around. So, if you see anything abnormal, it's not the time for boil. A war is not No, no, no. None of those should be there. And again, there should be no pain because it's outside of your
34 menstrual cycle. The hormones have come down. If you notice anything abnormal, you want somebody to know. One of the things we realize is during your menses, you may see some cyst in there. They're coming from the hormones. Once your menses goes away, they disappear. So, you will know yourself every month that Okay, this thing is normal. >> What if it is like a big lump inside? I mean, it's kind of So, it's kind of Okay, people will make People get confused that is that a cancer lump or cuz there's this huge lump inside. Is that normal? >> Yes, that is normal. That's part of your lobules. That's where actually the milk-producing cells, that's where they reside. So, that one is normal. >> Okay, we're going to wrap it up very quickly. Yes, Chris. Yes, ma'am. Where's the mic? >> Please, I have a consent question. So, it's so It's worrisome. There are some
20 people um that are single and they're like from like 35, 40 to to um 45 and they don't have a man. And some are under the Christian um um seedom that you don't have to have sex before marriage. How do they How can they cope? How will they be able to manage um this cancer risk? >> Yeah. It's so concerning to me, please. >> No, that's that's a you know, we live in a society where we are both religious and cultural. So, we need to be mindful of that. In physiology, if you don't use it, you lose it. If you don't use it, you lose it. So, we need to keep that in mind. More important question for that single lady
05 is take a step back. When did you start your menstrual cycle? And when are you expected to each menopause? That length of year determines your risk of sexual cancers, breast, ovarian, cervical. What we recommend is for a young girl to start menstrual cycle late. We don't want the 9 10 11 12. We actually prefer 14 15. If she starts at 9 and she start developing breasts, she's exposed to estrogen. Almost all these breast cancer sexual cancers are estrogen driven. So you don't want that. So what's the take home? If you have a girl or you know you started early, start doing your screening early. >> At what age? >> No, she have not asked her question now. So what does a man woman like that do? >> You want me to go there?
50 >> No, go there. Okay, don't go there. Don't go there. Don't go there. Okay. Okay, go there. We are >> Look, so so to answer your question directly, that woman's body needs to be serviced. Unfortunately, she doesn't have a man, but there are other ways like vibrators. You want to use that. There is nothing wrong with >> suck your breast now. Vibrator cannot suck your breast. >> No no now, remember Okay, so the essence of sucking breast is to get you turned on. So the other six seven holes in your body can be lubricated. Vibrator can do that for you. Because to all women, let's be frank. There is no point making love to a guy that cannot get you to reach orgasm. I'm being honest with you. Don't settle for a 2-minute man. When you reach
37 orgasm, your body releases oxytocin and releases a lot of endorphins that makes you healthier compared to someone who's not sexually active. >> [applause] >> Thank you very much, sir. >> You're welcome. >> Please, I have two questions, sir. >> Yes. >> I want to ask the cause of a young ladies in their 20s with busty breasts and their inverted nipples. What causes inverted nipples for them? And my second question, sir, how about the the elderly ones that no longer menstruating? How do we when do we check our breasts? Okay. >> No, excellent question. So the question is, let me take the second one first. Someone who is menopausal
22 >> Yeah. >> and now how do how does she check her breasts? >> Yeah. The cutoff for us is 70. If you are 52, 60, continue to do your mammogram. Because you can you are still getting some exposure to estrogen just at a reduced level. >> Yeah. >> Not only that, females who are now close to menopause, your ovaries are producing testosterone just like men. So, you need to do this mammogram and do your regular screening even though you are no longer having the monthly flow. Okay? And your first question about the young lady in the 20s that now >> has this abnormal nipple stuff. >> Yes. >> Yeah, there are big same patients where we normally use to suppress a man's was on a medication with young ladies like the of 12. The boy has breasts as if he's a woman. And so, the same thing Now, at the age of 12, the boy has teenage breasts as if he's a woman. And
08 so, the same thing happens with young ladies like that just outside of teenage life as if he's a woman. And so, the same thing happens with young ladies like that just outside of teenage life. So, you need to go back to your primary care doctor. There's nothing wrong letting the doctor know you want to get a second opinion. This thing is not normal. So, when Dr. A tells you one thing, go to Dr. B just to compare. >> Okay. Any other important question? Yes, please. Go ahead. Was that okay? All right. Okay. Anybody else? Okay, final question. >> questions on that. >> Debbie Debbie Okay, we have a question. Go ahead. After you then, Debbie, and then online and we're done. Okay. >> Thank you, Dr. Abbasi. Okay, my question is um for those that life happened to that been enjoying this sexual um thing
55 you've been talking about and the sucking part of the breast and all of it. What do you expect them to to do? And people like that I mean, people like them does not like the vibrator you're talking about because they don't have blood in their veins. So, what what's your advice? >> Yeah, I have another question. >> Yeah, it's she's talking about someone that life has happened to. Maybe they become a widow or maybe they went through divorce. And now they don't have the man that was doing the job. >> Okay. Okay. >> In in my field of work, I think you know, it's a game of numbers. There are more single women than men. And we're still going to get to the area that we may not like. Every woman in this audience has a guy that she has a crush on. And you are expecting the guy to come
41 and ask you out. Where you could have sent him a silent DM. There's no shame in this game. So, there's no point for you just waiting on the guy to initiate all the time. You see the guy that meets your spec, you have your level of respect, you're not going to go below that. Send him a DM. What you do in your privacy is your business. There are guys available for this. >> Hey. Debbie. Wait, wait, I don't understand. Wait, wait, wait, wait, wait. I don't Let me understand Let me Let me see if I understand what he said. No, no, teacher, don't teach me nonsense. Are you saying that as a single person, if I like maybe Sadiq now,
27 I'll go to Sadiq this message. So, so does Wait, is it that men are available for this? Meaning that I can get Sadiq to come and do the job for me. Is that what you're saying? >> Let me be honest with you. We've seen We've seen on social media where the girl was just saying, "Hello to the guy." And the guy's next thing is, "Oh, are you available?" And the guy said, "Yes, yes, yes yes yes yes yes." There's nothing wrong with that. >> Okay, let's move on. Yeah, Debbie. >> [cheering] >> Okay, good morning, sir. >> Good morning. >> I'm Debbie. Um please, there's this I used to hear this thing that if like a lady now that has that has done breastfeeding, maybe like person don't born finish. >> Yes. >> I want to go for breast lift. They say that once you do that breast lift, your your boobs won't have that sensitivity again. Is it true? Yes. Thank you, Daddy. That's the my question because I
12 want to go and do breast lift, so >> absolutely. Especially those who are going with implants. If you're just doing lifting, they still preserve some of your nerves around the areola. But if you're going for insertion, most of the time because it's surgical, and they're creating space for that silicone to come in, some of the nerves will be damaged. >> Mhm. So, like me now, I want to do a reduction and a lifting. I want to be like a small, you know, just, you know, just so that I can so that I can wear it. MY DESIRE IS NOT TO WEAR BRA AGAIN. I don't want to wear bra again. I'm tired of wearing bra. I want to stop it. Something like that. So, someone like that wants to do a reduction. Is that okay? Will I still have the nerves? And if I now do a firming, will I still have the >> Yes, so you want to see a specialized plastic surgeon. >> Okay. >> So, it's going to be a two-step
57 procedure. They might not do the two the same day. >> Oh. >> You might do your reduction and then they bring you back and then they do the firming. >> Ah, okay. All right. God, you should warn her now. What she want to know now? Say, "Oh, your final comments on online online online." >> All right, very important. This person has been particular about this. Pearl Toya says, "Good afternoon, doctor. I have a niece who doesn't have breasts at all. Please, what can we do, sir? They've gone to several hospitals, but can't find a solution." First question. Second question is, "Good afternoon, doctor. Please, doctor, I have encountered a woman that doesn't have nipples. Is this normal?" >> Ah. Crazy. >> Third question. Can she still >> first, one now. Uh-huh, what Go ahead
42 now. Can I can breastfeed it? >> All right, can breastfeeding cause breast cancer? Good. Excellent question. The last one is the easiest. There has not been any correlation between breastfeeding and cancer. Breastfeeding is actually recommended. It helps a woman's self-esteem. It helps your baby more than anything because you transfer antibodies. And the woman will end up also losing weight. It's one of the functions of it. The first part about a woman that is flat-chested, this is hormonal, honestly. Go to your primary care doctor even before you see the specialist. You want to check your testosterone level as a woman and you want to check your estrogen level. It's the estrogen that determines this most of the time. Men and women were given breast tissues. So, even though it's breast awareness, one out of eight women will develop
28 breast cancer. One out of 800 men will develop breast cancer. Yes. So, it's all hormonal. And the correlation is they're all driven by hormone. So, if you see someone who is 18, 19, 20, and they're flat-chested, trust me, check their hormonal level, especially the estrogen. And we have medications for it. Yes, it can be fixed. >> Then the third one of What's the third one? Nipple that is no nipple. >> Yes, that's abnormal. So, now plastic surgeon can actually create nipple for you. Just like we do it in the cases of people with a mastectomy because of breast cancer. We create it and they can Yeah. >> Imagine that. Please Dr. What is because of breast mastitis? >> So, anything that ends in itis is
13 inflammation. >> Okay, inflammation. >> So, we see this a lot in women who are breastfeeding because your baby's mouth is full of bacteria. And some babies, even though they don't have teeth, the way they grab onto the nipple, they can cause a little bit of skin abrasion, and their bacteria is falling to you. So, you have inflammation. So, at that time, stop breastfeeding, apply warm compresses on it. You can pump, and the inflammation will go away. >> Okay. I think that's all. I've had enough about breasts, I should say. Uh-uh. Dr. I always invite you to my kitchen, but I don't think I want you to go to my kitchen again. Our breasts are too awkward. But, thank you so much, Dr. Abbasi.
