27 and do not place any material to obstruct manual call points, fire extinguisher, hose rail or safety charge. Do take note that you are only allowed to smoke at certain areas under yellow umbrella. For Fab 10W, it's at main entrance and main dock. For Fab 10N, it's at car park area beside loading bay. There are three designated umbrella points at FAP10M for a team member to use during extreme weather conditions. Please return after use. To minimize potential chemical exposure, sliver shirt, singlet, skirts and shorts are not allowed at the folding places. All PPE designated zones, warehouse,
13 main docking area, cup building and gas yard. Facilities work permit is required to ensure works are carried out safely and prevent interruption to the process. It is not required for routine job that is administered by FAB10's SOP and RA, activities to be performed by FAB10's team member or approved in-house vendor except confined space entry, hot work activities, working at height and new process tool assembly. Now we touch on fire safety. Take note that no staging in front of fire safety equipment is allowed. Equipment or part should have a minimum gap of 0.6m away from fire installation point and strictly
58 no object is to be placed in front of fire access panel marked with ripped crossbox. Poor electrical installations and faulty electrical appliances can cause fire. Use proper PPE or tools when working near electrical source. Do remember to use ERCBE for all portable tools as well. You shall implement a control of hazardous energy program that meets or exceeds Micron's corporate EHIS control of hazardous energy standard and local EHIS legal requirements. Micron reserves the color rate for lockout takeout logs and tags used for the control of hazardous energy. Secure materials on trolley and do not overstep to greater than 1.2m high because you cannot see what is in front of you. Gas cylinder should be
47 positioned upright with top and bottom chains secured to prevent toppling. Protective caps should be installed and no transportation without caps. Do remember to inspect cylinder trolley before use. Lifting appliances with safe working load more than 150kg should be examined and load tested by approved personnel annually. Ensure inspection certificate is available and valid, do remember to submit a copy to HHS before actual move in. If you need to enter a confined space, do ensure that you are properly trained and follow confined space entry requirements such as pre and post entry gas check, purge or clean thoroughly before entry, and constant circulation of fresh
34 air through continuous ventilation. If you need to perform hot work, take note that hot work permit approved by site EHS is required. If possible, perform offsite or away from combustible material and a fire watch shall be appointed throughout the duration of hot work, during breaks and for at least 3 hours of continuous watch after completion of the hot work. Proper housekeeping is important. Remember to clear all waste materials, keep passageways and aisles clear Store all equipment and tools in an orderly manner, wipe and dry wet surfaces, do not store spare parts in office area, and dispose material in disposal bins and dumpsters.
21 Proper cable management is important to eliminate tripping hazard. For storage and transportation safety at Fab 10W, do remember that height of stacking should be less than 1.5 meter Maintain a minimum gap of at least 1.6m for storage near CMP sliding dock. Full body system when using cargo lift. And no item can be left inside the cargo lift for more than 20 minutes. Safety shower and eyewash stations are strategically located throughout the site. Please use them only if exposed to chemical. Before you use a safety shower, remember to call 636-07-111
06 for FAP10W or 66370111 for FAP10N. Enter fully rounded and start water flowing. Draw the curtain and cautiously remove all garments and shoes. Flush with water minimum 15 minutes. For hydrofoil acid exposure, flush for 5 minutes only. For eyewash station, Call 63607111 for FAP10W or 66370111 for FAP10N. Flush ice with water minimum 15 minutes for HF exposure 5 minutes. ERT will then arrive to provide further assistance. They will provide calcium gluconate
56 solution or gel for HF exposure. Personal protective equipment is important to your safety. Remember to wear safety helmet, safety glasses, earplugs, safety shoe whenever you need. At level 1, you should wear safety helmet, safety glasses or goggles and safety shoes. 20:45 Remember to wear chemical gloves when handling chemicals, abrasion resistant gloves when handling quads or broken wafers, chemical resistant coat for any intrusive chemical work. Now we'll touch on chemical safety. There are different classes of chemical example, flammable, firephoric, water reactive, etc. There are a few methods of detection, example, continuous monitoring or air sampling. Here are some exposure and effects to chemical. It can be through, injection, inhalation, injection or dermal. And the effects are either acute
31 or chronic. Request approval using SIDA prior to bringing chemicals on site through Micron Host. When handling chemicals, remember to read SDS or labour before using a chemical, follow the safe work procedures and take any special precautions, wear proper PPE, dispose chemical according to SOP and SDS, and chemicals should be stored in original container with original label or proper labeled secondary container. Chemical Safety Storage of Chemicals All approved chemicals should be kept in proper storage area. All flammable chemicals to be stored in
18 factory approved flammable storage cabinet. Yellow cabinet Exceeded and caustic material to be stored in compatible material cabinet with leak-proof silk. Maintain and humanitarian use of all chemicals on site and ensure the approved chemical stock on site is within permitted maximum quantity and pasted outside the yellow cabinet. For any accidental spillage at FAT10W, remember to use the emergency spill kit. Provisions in the spill kit include chemical gloves, absorbent pads, barricade and masking tapes. Spill kits are only intended for emergency spill and contact KMG office at 636-07-199
08 before use. The locations of emergency spill kit at FAT 10 WA Level 1 are Beside Acid Caustic Store Beside Sorbon Dispense Room Beside Caustic Dispense Room Beside Caustic Dispense Room The locations of emergency spill kit at FAT 10 WA level 1 are, beside estate or caustic storeroom near pillar 1B-X, beside solvent dispense room near pillar 9-H, beside caustic dispense room near pillar 8-X. The locations of emergency spill kit at FAT 10 WA level 1 are,
54 Beside chemical leaf near pillar 33 slash B3. Add chemical dock 2 near pillar 44 slash A4. And beside acid dispense south wound near pillar 33 slash N2. Hazard communication is important. You can identify hazardous material by reading its labels and SDSs. Follow general safety precautions for safe storage, usage and disposal and know the appropriate context. Below are some of the examples of labels for safe condition, fire safety, mandatory action,
39 prohibition and warning. Below are some of the globally harmonized system labels. Examples are oxidizer, explosive, corrosive gases under pressure, etc. Corrosive chemicals are chemicals with pH value less than 2 for acidic or more than 12.5 for caustic. The acute hazards are burnt skin and eyes, irritated nose, throat and lungs. One of the examples is hydrophobic acid. It is a clear odorless liquid, sensation from contact delayed, absorbed through skin, attacks bone, and causes intense pain hours after contact. Carcinogen is any substance, radionuclide
28 or radiation that is an agent directly involved in causing cancers. Example is arsenic particles which is an implant by-product. Now we'll go through tetramethylammonium hydroxide, DEMA safety. This is an overview of a specific hazard. This is not a substitute for comprehensive training. Your employer should provide the following. Training about hazards of chemical you work with and task specific training include hazards and know how to avoid them. You can recognize area with DEMA by this size and warning tip. Now we will go through some of the characteristics of DEMA. DEMA solution is a clear colorless to yellowish liquid. DEMA may be used
14 in solutions that have a different color or appearance. T-marz solutions may have no odor when pure or may have a slight amine featuring odor. T-marz solution may be slippery, do not touch a mystery liquid to find out. Concentrated T-marz solutions are very strong bases with pH above 13. pH changes with concentration. T-marz solutions are not volatile and are not anti-inhalation hazards unless in the form of fine airborne droplets or mist. Heating Tima or welding a pipe with Tima in it creates other vapours that are less hazardous but still must be avoided. Chemicals containing Tima are a serious safety and health concern in the semiconductor industry. Here is
02 a basic description of the hazards. Concentration above 1% should be considered a potential systemic health hazard, particularly when larger surface area of the skin are exposed. Concentration above 2% can handle the body by burning the skin. Absorption through the skin is the primary route of exposure of main concern. Tima exposure can cause the following. Corrosive burns to any contacted tissues. Toxic effects happen by absorption of Tima through the skin. As the concentration of Tima, amount of skin exposed, and time increases, the toxic effects get worse. Exposure is potentially fatal.
51 Exposure may result in intense burning of the eyes, nose, throat, lungs and skin. If you are exposed to TMA or a chemical containing TMA, immediately get in an emergency shower or immediately flush your eyes in the eye wash. Stay in the shower. Call the emergency response team immediately or if possible have someone else call the ERT. Wear the correct PPE and make sure it is rated for use with TMA. Always inspect PPE before use. PPE that has been exposed to TMA shall not be reused and shall be disposed of, with exception. Chemical boots, face shields, respirators can be decontaminated.
39 Read and understand the safety leadership of all chemicals you work with. Know the emergency number of your site. For lifting operation safety, you need to acquire a side lifting permit to work prior to commencing work. Summit an approved lifting plan if swing radius of crane overlaps a section of micron building. No lifting operation shall commence before the safety of persons and property below are ensured. For excavation operation safety, A copy of task-specific risk assessment, result of underground utility survey search results, and identification of shoring, bracing or sloping materials shall be submitted to the Micron EHS representative for review.
26 All excavations shall have daily documented inspections performed by competent person and shall be made available upon request. Working at height permit is required for work undertaken at height of 3 metres or more. Means of access and working platform should meet required standard and properly maintained. All open holes or leaving edges at height of 3 metres or more must be protected using fall protection. Area below overhead work shall be barricaded to prevent pedestrian traffic from falling objects. Use safety harness when working at height of 2 metres or more. Scaffles at micron shall be tapped, safe for use, after proper inspection by competent person.
14 Scaffold that do not meet minimum safety standard shall not be used and shall be retracted. Access ladder or equivalent means of safe access must be provided on all elevated work platforms. For ladder safety, ladders shall meet OSHA, NCIL, EN313, BS or JIS standard. Only use type 1 and above ladders that support up to 110 kilos and above. Only fiberglass ladders allowed, not wooden ladders. Utilizing top 2 steps of ladders is prohibited. Work areas with ladders shall be barricaded and body systems implemented. No more than one person working on the ladders.
01 Don't overreach or jump from ladders. Don't place over uneven or slippery surfaces. Always face ladder when ascending, descending and performing work. Respect ladder before use. Do not use damaged ladders. Persons on ladder should maintain 3-point contact at all time. All ladders must be registered. MarkleFab10 hosts need to submit and upload ladder checklist via F10WAH. Fab10 EHS will issue a sticker upon verification of the checklist. Validity of the sticker is for one year only. Micron Fab10 hosts need to re-inspect, resubmit and upload completed letter checklists every year. For ergonomic guidelines, remember to
52 maintain a neutral position when sitting or standing. Keep objects close to body when lifting, carry and holding. Push rather than pull. Then reduce twisting by using legs. For stairwell walking and handrail requirements, walk slowly, do not run or double step. Ensure one hand is free to hold handrail at all time. Avoid distractions such as phone composition, text messages, etc. Do not block access to others by holding composition. Do not pass others on the stairs. Beware of slippery conditions report any injuries and report any stairwell issues. Materials may be carried as long as one hand
40 remains holding the handrail. Materials weight greater than 7kg or without a proper grid should not be carried in the stairwell. Use an elevator if arms are full and unable to hold the handrail. Complete risk assessment if elevators are not available for material handling. For road safety, No squatting on the road shoulder and parking lots near a smoking area. Observe all traffic signages. Observe speed limit. For Fab 10, speed limit is 25 kilometers per hour. Do not cross gate barrier when it is closing. Always off engine and choke view during loading and unloading. Use path on the side of the
26 road and observe no stopping sign. Safer procedures for working in remote areas Implement body system for all team members, vendors, contractors who need to work alone in remote areas or areas where no one can hear you even if you call for help. If team members are required to work alone due to operational needs, the following shall be complied. For Micron team members, notify your supervisors if you are proceeding to work alone in remote areas Ensure to call back to your supervisors to report on your status at one hour interval. Call and inform the supervisors upon completion of work alone and remote areas.
12 For Micron supervisors, upon enlargement, ensure your team members contact you at one hour interval and upon work completion. If team members were supposed to call back but did not, check with security if the person is on site. If personnel is on site, Micron supervisors to search for the person. If unable to find a person within 15 minutes, Micron supervisors to notify security to search for the person. For Micron host, notify your vendors or contractors to ensure two box meeting before work start and wrap up meeting after work completion to account for the presence of all personnel. Notify your vendors, contractors to conduct roll call before commencing work. If personnel who is supposed to turn up but did
01 not, check with security if the person is on site. If personnel is on site, vendors, contractors, supervisors to search for the person. If unable to find the person within 15 minutes, vendors or contractors, supervisors to notify security to search for the person. Now we'll touch on hazardous waste management. For solid waste disposal procedures, place solid waste in hazardous material bag and seal with tape. Double bag and seal the second bag properly with no opening. Wear chemical gloves when handling bag. Attach completed waste travel form onto bag
47 and place inside chemical leaf or transport to KMG personnel. For liquid waste disposal procedures, drain tank or container shall not be filled to the top allow space for expansion. Ancient lead is tightened before removal from machine. Wipe residual liquid on container surface. Wear chemical gloves when handling container. Attach completed waste travel form onto container and place container inside chemical leaf or transport to KMG personnel. Now we'll go through hazardous waste management for Fab10N and X. The bin in the picture is for solid waste not contaminated with chemical only, example cleaning paper or plastic.
35 Please use the red colour bin for flammable contaminated waste. Maintainance parts contaminated with solvent shall be double bagged with hazardous bag, labelled with waste traveller form and disposed into the 660 litre solvent bin. Please use the yellow colour bin for acid and base contaminated waste. Maintainers parts contaminated with acids are based, shall be double bagged with hazardous bag, labelled with waste travel form and disposed into the blue colour 660L solvent bin. Please use the IPA waste bin for IPA wipes at wipe down station. For arsenic waste disposal, please use the respective arsenic contaminated acid, arsenic contaminated solvent or arsenic
22 contaminated magnesium waste bin. For lamps and IT equipment disposal, dispose bulk or lamp consists of chemical into the 200-litre equipment lamp disposal drum bin. Used I-Line high-pressure mercury lamp must be placed in the clamshell and fit into the lamp card. Contact IS department for disposal of IT equipment. For batteries disposal, dispose standard alkaline batteries in general waste bin, milk acid, lithium nickel metal hydride and nickel cadmium based batteries are toxic industrial waste. Disposed into hazardous waste bags, labelled with waste traveller form and disposed at decon or chemical room. For SOD waste disposal,
11 waste is made out of spin fuel and HC100. Waste is flammable when come in contact with water, acids, alcohols or amines. Remember to prepare risk assessment for SOD related work. Wear appropriate PPE. Handle SOD waste under air exclusion environment. Do not handle SOD waste in open air or hand pouring of SOD waste. Now we'll go through raised metal floor safety. Requirements for raised work floor are barricades, appropriate signages and towel pullers, for example, suction cup lifter for flat surfaces and double J lifter for perforated surfaces. For hard barricade, struts railing
01 attached to raised floor surface for multi opening. For single tile hard barricade, use barricade system for single tile opening. For soft barricade, proper safety codes with poles should be used and attendant should warn surrounding team members. For a proof hole cover, material that meets standard strength and construction requirement is needed. Now we will go through AMHS Safety. Automatic Material Handling System, AMHS, facilitates automatic transportation of hoops from one location to another. It has a travelling speed at about 200 meters per minute when it's travelling straight and 60 meters per minute when it's turning. It has a potential physical hazards
50 because of the heavy overhead hoist transport vehicles with load. There's a height clearance from fall difference for both FAP. For FAP-10W it is 2.5m, for FAP-10N it is 3m. At FAP-10W a red line at 2.5m height is a warning sign, but at FAP-10N there's no red line. Load permit must be applied for work above 2.5m at FAP-10W, or 3 meters at FAP10N at AMHS area. For FAP10W, please apply through FAP10M at the storm. For FAP10N, apply through alias F10MWS review. Please remember, downward check
36 sensor only scans the front of the load port. Do not sit, stand, lean or extend elbow at the side of the load port. Now we will go through some of the lesson learned For this incident, team member sat on a two-step ladder to observe a PM procedure. His finger was pinched by the ladder after it collapsed as the latch was not secured. The root cause is no awareness of safety concern for ladders less than 6 feet. The lesson learned is, step ladders shall not be used as tool and to ensure that ladder latch are properly secured before use. For this incident, team member suffered laceration on his forearm when the IRAM move up and hit his arm during tool recovery. The rule cause is, energy isolation instruction
26 was not followed for intrusive work into that tool. The lessons learned are, ensure that the knockout takeout procedure or energy isolation procedure established and implemented for any PM or CM work, especially intrusive work into any tool. Report any safety related interlock that are faulty or bypassed immediately to your supervisors and managers. For this incident, Team member accidentally creates his left forearm against the screw on the scaffold clamp when performing scaffold inspection. The rule course is, sharp edges due to nicks on scaffold bracket screws. Lesson learned are, verify open sharp edges along the assessed way for scaffold when doing scaffold inspection. Refocus on physical hazards presence on site at similar environment setting
11 to team members who are responsible for scaffold inspection, scaffold users or those working in areas with scaffold. For this incident, CMP PE technician experienced acute back pain when he bent forward to mount a polisher head. Prior to that, he lifted a used polisher head. This motion could have triggered the back pain. He was sent to A&E department. The root cause is incorrect lifting posture. The lessons learned are procedure can be further improved to include techniques on proper lifting. Jib can be redesigned to eliminate excess lifting. Use of back support belt during lifting. For this incident, team member was preparing sampling bottles with 2% HF. She felt some pain in her middle finger tip but continued to work. Pain became unbearable after 45 minutes
00 and team member headed to the clinic. Her finger was flush with water and followed by a calcium glucanic application. She was taken to A&E for further treatment. The root cause is, gloves could have punctured during activity. The lesson learned is, check gloves before usage. For this incident, Efficient equipment technician got his booty stuck on a part near the boat elevator, lost his balance and injured his head. The root cause is restricted space and low head room inside the tool, incorrect booty size or booty clips are loose. Lesson learned is implement bump caps when performing PM or CM, ensure booties are strapped, snapped and not loose. For this incident, team member was inspecting
45 the back portion of the tool with her colleagues. step into the cut-out hole and injure herself. The root cause is, clear acrylic cover was but placed on the floor opening, inadequate barricade and warning signs. The lesson are, floor openings or cut-out holes should be barricaded or covered appropriately to prevent injury. Environmental inspects fed them use natural resources such as chemical, gas, electricity and water due to the manufacturing process. noise, greenhouse gases, air emission smell, general waste, trade influence, discharge of hazardous waste has been produced. The environmental impacts from this are water pollution, air pollution and land pollution. FAP10 have all control measures in place
32 to ensure we do not pollute the environment. Refer to below site for significant expect at FAP10. It includes electricity usage, water usage, chemical and gas usage. Safety Requirements for Working on Clean Room Sealing Panel Only one person is allowed on 1.2 x 1.2 meter ceiling plank panel. The total weight for one person and equipment shall within their maximum safe workload SWL within 100 kg. A person shall work on the ceiling plank panel and look out for panels that are not seated properly. Beware of the trip hazard of the ceiling grid as shown in the picture. Always maintain a 100% tie-off.
22 Do not cross over the yellow chain, the red-white tip boundary. Change its position 1.2 away from open edge to prevent team members reaching the edge and fall. Tougher rated ceiling panels are not allowed to stand and walk. Modified ceiling panels including cut-out panels are clamped down in position and labelled do not step The person shall do a visual inspection before work starts. Work must not start if the seating plane banner is not seated properly. Fan filter unit FFU safety. Do not stand on road of FFU during installation works. This is because FFU not installed with filters are only supported by two extension roads
11 which cannot take a person's weight. Do not walk Step stand on the center of FFU, instead stand on the edge of the FFU to avoid any fan damage. Work and height permit enhancement New part 1B and new part 4A has been included in the work and height permit enhancement. Anyone accessing ceiling panel are required to attend safety training on work above the premium ceiling panel. Sealing panel undergoing modifications and installation phase are required to be accessed by a competent person. Upon job completion, site supervisor or requester are required to verify the seating panel are seated properly after work. Autonomous Guided Vehicle
